Commitment to Learning / Declaration 
 

Each CEAP student is required to accept the "Commitment to Learning / Declaration"  that is contained on the student application form. The terms of this agreement serve to define and clarify several important points pertinent to the student's relationship with CEAP, the staff and learning resources:

The CEAP Commitment to Learning / Declaration states the following:

Terms of Enrollment: The Applicant confirms that the information in (the Application) form is accurate and complete, that the Applicant has the legal right to enroll the student in CEAP, and that the Applicant understands that no materials will be forwarded until fees and deposits (where applicable) are paid; that work required will be completed and returned as directed; and that enrollment remains valid for one year.

Commitment to working together: The Applicant confirms a commitment to working together with CEAP in completing the coursework agreed to by all parties during the normal academic calendar (unless otherwise negotiated). Working together is defined by consistent activity in courses (unless previously agreed to, consistent activity is defined as weekly submissions according to the personal education plan of the student) and continuous communication with the CEAP teachers. The Applicants also agrees that all the assignments submitted will be his/her own original work.

Consequences of Inactivity: The Applicant understands that if the student is inactive, he/she may be withdrawn from course(s) or the program. The Applicant agrees that withdrawals from courses will only be granted under special circumstances, at CEAP’s discretion, and that in cases of non-completion, a final mark of F will be assigned.

Terms of Resources Borrowing: The Applicant agrees that textbooks and other resources will be kept in good condition and returned when the Applicant completes or discontinues the course(s). If the materials are not returned in good condition, then the Applicant will forthwith pay the replacement cost thereof or will forfeit the deposit. Textbooks must be returned within eight weeks of course completion or withdrawal from CEAP, or an invoice for the full replacement value will be issued for any textbooks not returned, minus any deposit on file.

Use of CEAP Technology Equipment: If using CEAP computers and/or online services, the applicant agrees that the Internet access only be for educational and lawful purposes, according to Internet Use Policies defined by the School District.

CEAP Access to Previous Records or Student Information: The Applicant hereby authorizes the release of all previous records (including confidential special education records/file, if applicable) to the Collaborative Alternative Education Program, and authorizes Collaborative Alternative Education Program to report to schools, school districts, or post-secondary institutions where records exist.

I verify all the above information as correct.  I acknowledge that I am NOT taking this course at the same time with another school. I will notify Collaborative Education Alternative Program of any future changes to my personal information. I understand that textbook/course deposits will be returned at a rate of 100% less damages.
